The rise and fall of endothelin receptor antagonists in congestive heart failure.

چکیده
S ince its discovery in 1988 [1], the role of endothelin in cardiovascular disease has been intensively studied. Activation of the endothelin system is considered part of the neurohormonal response in congestive heart failure. Endothelin is produced by vascular endothelial cells and has potent vasoconstrictor effects [2]. Elevated endothelin levels induce adverse cardiac remodelling and cause progressive aggravation of congestive heart failure by influencing loading conditions of the heart and by reducing coronary flow; endothelin also has direct (toxic) myocardial effects [2]. Several clinical studies have demonstrated that higher plasma levels of endothelin-1 correlates with the degree of haemodynamic and functional impairment, and worse prognosis in heart failure [3–6].

Endothelin receptor blockade in congestive heart failure.
Heart Failure To the Editor: Of the many new therapeutic agents for slowing the progression of congestive heart failure (CHF), other than antagonizing the renin-angiotensin-aldosterone system, blocking the endothelin receptors seems to be most promising. The two latest reports published in Circulation1,2 render further support to this exciting concept. Endothelin may contribute to the symptoms ...
متن کاملThalassemia Associated Pulmonary Hypertension
Cardiac disease is the main cause of death in both forms of thalassemia; thalassemia major (TM) and thalassemia intermedia (TI). Pulmonary hypertension (PH) is one of the cardiopulmonary morbidities with high mortality that, if not treated, may trigger right-sided heart failure and premature death. PH is defined as a mean pulmonary artery pressure of ≥25 mmHg at rest or ≥30 mmHg during exercise...
متن کاملEnhanced endothelin-1-induced contractions in mesenteric arteries from rats with congestive heart failure: role of ET(B) receptors.
Studies of congestive heart failure (CHF) in man and in experimental CHF have demonstrated elevated circulating levels of endothelin (ET). In order to examine whether there are concomitant ET receptor alterations, the vasomotor effects of endothelin-1 (ET-1) and sarafotoxin 6c (S6c) were examined in endothelium-intact and -denuded isolated mesenteric arteries from rats with CHF. CHF was induced...
متن کاملEndothelin receptor antagonists in congestive heart failure: a new therapeutic principle for the future?
Congestive heart failure (CHF) is characterized by impaired left ventricular function, increased peripheral and pulmonary vascular resistance and reduced exercise tolerance and dyspnea. Thus, mediators involved in the control of myocardial function and vascular tone may be involved in its pathophysiology. The family of endothelins (ET) consists of four closely related peptides, ET-1, ET-2, ET-3...
متن کاملEndothelin receptor blockers in cardiovascular disease.
The endothelin (ET) system is comprised of 4 active ETs, with ET-1 being the predominant isoform in the cardiovascular system. Because of the potent vasoconstricting and mitogenic effects of ET-1 and its involvement in various cardiovascular diseases, blockade of the ET receptor has received considerable attention. ET receptor antagonism has been demonstrated to be beneficial in patients with p...
